How to Fill It Out, Section by Section

When to use this sheet

Use when the main hazardous waste consignment note has insufficient space for waste entries. It supplements the main note and is not a standalone movement document. It includes three corresponding copy pages.

Header – Link to the main note

Copy the complete consignment note code, including any additional letters, exactly from A1. Fill in Sheet __ of __ consistently. Identify the correct copy recipient. Tick the continuation-sheet box on the main note.

Part B – Extra waste entries

For every additional EWC, enter the full waste description, six-digit code, accurate kg, components and concentrations, physical form, hazard properties and container type, number and size. Do not duplicate a main-note quantity in a way that double-counts it. Keep entries unambiguous across all sheets.

Part B – Transport details

Add the EWC, UN number(s), proper shipping name(s), class(es), packing group(s) and special handling requirements for the extra waste entries, where applicable. Check the information against the waste classification and dangerous goods requirements.

Part E – Extra receipt entries

The consignee enters each EWC received, actual kg, acceptance or rejection and the R/D operation for the extra waste entries. This table supports the main Part E certificate, which is completed on the main note.

Attach and check every copy

Attach the relevant continuation pages to the producer/holder/consignor, carrier and consignee copies. Check all extra information before the main C and D signatures. Do not use this sheet to alter certified details. Read the official guidance for unusual movements or rejected loads.
